The measures you’ll be asked for
The CMS HCBS Quality Measure Set now includes community inclusion, social connectedness and transportation. Tailored Plans will need to report them. Today, most of that data comes from a yearly survey.
Community Inclusion
Does the person go out to shop, eat, have fun and take part in groups?
Social Connectedness
Does the person have friends and people to do things with?
Transportation
Can the person get where they want to go, not just where they need to go?

The proposed 2028 set asks if people are as active in their community as they would like.
A service only counts if someone delivers it
Community Networking is in the NC Innovations waiver. But if no provider near a member offers it, it stays on paper. Groulpi Guides add Community Networking capacity, starting with the homes in our pilot.
We want to talk with Alliance, Trillium and Vaya Partners about network admission and how this fits alongside Residential Supports.
Authorized, not delivered
Members can have Community Networking in their plan and still have no one nearby to provide it.
Guides where members live
Trained Guides work alongside the home’s staff, so outings happen without leaving the house short.
Outing data you can actually use
Every outing in Groulpi leaves a record. Insights rolls those records up by member, home, provider and county, so quality teams don’t have to wait for a yearly survey.
